Instructions for New Clients of Kent Berry

Schedule:  I currently see clients on Wednesday and Thursday evenings at 6 and 7pm and on Sundays between 1 and 5pm.  If these appointment options are compatible with your schedule, please follow the below instructions to create an account and schedule your first appointment.  

Financial Information:  I am a provider for Blue Cross Blue Shield, Cigna, United Healthcare, UMR, and Medcost insurance panels; and I am considered out-of-network for others.  Contact your insurance company for information regarding your financial responsibilities.  Confirm what your copay is and the amount of the session charge you will be responsible for if you have yet to meet your deductible. 

My fee is $135 for the initial diagnostic session; $125 for couple or family therapy sessions; and $110 for individual appointments;  Sessions last between 45 and 60 minutes.   You may safely keep your credit-card information saved in your "Cllient Portal" account;  The portal utilizes an encrypted server that is Payment Card Industry (PCI) compliant.  Co-payments and/or deductibles  will be charged to your card after a session is completed.  If you do not have a credit card, I am glad to accept a check or cash.

If you will be using your EAP benefits from your employer or you have received a pre-certification letter from your insurance company, you may fax this information to (336) 505-5483 or bring it with you to your first appointment.

If you ever need to cancel or reschedule an appointment, you may do so via the Client Portal or by leaving me a voice mail.   However, canceled/rescheduled appointments that are made less than 24 hours from the time of the appointment are subject to a Late Cancellation Fee of $35.

Parking:  When you pull in to the driveway of the church, park in the lot to the right side of the sanctuary.  Then walk up the side walk to the 2nd door on the right which is a stairwell entrance.  The bench inside the door is where I will meet you after I finish with a prior appointment that I may have.   There are restrooms at the far end of the hallway and a water fountain as well.   My office is just around the corner from the bench.  If the door is open, always feel free to come in and be seated.  If the door is closed, it will mean that I am still meeting with another client and I will come and greet you in the waiting area.  Water is also available in my office as well. 

Registration ProcessIf you experience a problem with the below registration or scheduling process, you may email me at or text my mobile phone at (336.991.6620). 

  • Right-Click on this Client Portal link and select "Open link in a new tab" or "Open in a new window" so that can view these instructions while creating your account. 
  • Click the "Register as a new client" button on the Client Portal window

(Note:  In the future you may use the "Client Portal"  menu bar button to update your account, make a payment, or schedule an appointment. 
  • Enter Name, Date of Birth, and Email Address, and click "Continue".  
  • Confirmation Screen appears
  • To activate your account and setup your profile, click the "visit our site" link in the received email. 
  •  Enter your birthdate and press "Continue" Button
  •  Create Username and Password.

Note:  You will use these credentials for future visits using the above "Client Portal" menu button.

You will be able to reset your password should you ever forget
  • After (1) scrolling and skimming the "Terms of Service", (2) check the box "By using this software...."  which may automatically save and open your Dashboard.  If not, then (3) click "Continue" which will then open your Dashboard.
On the form, click the labels boxed in red  to complete the following tasks :     

    1.  "COMPLETE YOUR BIOGRAPHY"   (Clicking on the Orange Section headers will Expand/Collapse the sections for you to complete.  Click the blue "Save" button located at the bottom right hand corner of the screen to save your entries. )
    B. "My Docs & Forms"  (There may a 10 second delay before forms appear on the screen)
        (To Sign the Documents, type your legal name in the field below the form and then click the "Sign Document" button on bottom right of screen.  
        1)  Electronically Sign the "Authorization for Electronic Communication"
        2)  Electronically Sign the "Informed Consent Service Agreement"
        3)  Optional:  Download the "Modify Authorization for Electronic Communication" form, complete, sign with pen and bring to the first session.
        4)  Electronically Sign the "Notice of Privacy Practices"
        5)  View the "Professional Disclosure Statement and Biography" form.

    D.  Schedule New Appointment   (Text me at 336.991.6620 if this is not functioning correctly)
Clicking the "Expand All Sections" or the the Orange Section headers will Expand/Collapse the section.

IMPORTANT: You must click the blue "Save" button located at the bottom right hand corner of the screen to save your information.
